Case Management
Our Case Managers provide individualized co-ordination of medical and rehabilitation needs based on findings and recommendations obtained through regulated health care professionals. Case Management then utilizes this information to implement care plans to best suit our unique client needs. Our Case Managers then oversee, co-ordinate and manage ongoing care of our clients. Our Case Managers have a deep understanding of the health care system, , excellent communication skills, organization and team approach.
Social Work
Our Social Workers are regulated and accredited health care professionals who provide direct services and individualized therapy to our clients and family. Our Social Workers support our clients with adapting to their accident related injuries and challenges they face as a result of the accident through counselling and advocating.
Occupational Therapy
Our Occupational Therapists support our clients by developing individualized skills, supporting goals, improving physical and emotional abilities and support with adapting the clients environment to best suit their needs. Specifically, our Occupational Therapists focus on improving our clients ability to perform everyday tasks, participate in meaningful activities sand regain independence.
Rehabilitation Therapy
Our Rehabilitation Therapists support our clients with independence based on their needs which include community involvement, medical and personal appointments and overall personal goals. Our Rehabilitation Therapists support our clients to maximize independence of our clients personal, social, vocational, and physical specific needs as outlined by the team.
Enhanced Personal Support Services
Our Enhanced Personal Support Workers provide support services to our clients in home or in the community who require advanced medical and/or behavioural services.
